I want you to close your eyes and picture a scene that has played out in thousands of hospitals across the country every single night for the last fifty years. It is 3:15 AM. The lights in the labor and delivery alcove are dimmed. The only real illumination comes from the glowing green-and-amber screen of an electronic fetal monitoring (EFM) machine. On that screen, two jagged, continuous lines crawl slowly from right to left: the top line tracking the fetal heart rate, the bottom line tracking the mother’s uterine contractions. It looks for all the world like a seismograph reading during a minor tectonic event. A tired nurse, halfway through a grueling twelve-hour shift, glances at the screen, sighs, and tries to decide if that subtle dip in the baby’s heart rate is a temporary physiological adjustment or the first quiet whisper of impending brain damage.

This is the status quo of modern obstetrics, and to be completely frank with you, it is a chaotic, subjective mess. Despite the fact that we have poured billions of dollars into electronic fetal monitoring since its widespread introduction in the 1970s, the clinical outcomes have been shockingly flat. We have not seen a statistically significant decrease in the rates of cerebral palsy or hypoxic-ischemic encephalopathy (HIE) over the last several decades. What we have seen, however, is a massive, almost exponential spike in the rate of emergency Cesarean sections—many of which, in hindsight, were completely unnecessary. We are operating in a system that is simultaneously hypersensitive and tragically blind.
